What Is a GP Chronic Condition Management Plan (GPCCMP)?

A GP Management Plan (also known as a GP Chronic Condition Management Plan or GPCCMP), previously known as a Care Plan, is a detailed, and personalized management plan between you and your GP. It helps you manage your chronic conditions with the support of your GP and our practice nurses by providing a structured approach in the form of a detailed plan.

Your GP can assess your eligibility for a GP Management Plan during a consultation.  Typically, if you have a chronic condition that is expected to last 6 months or longer or you already have an existing chronic condition, you may be eligible.  If eligible, a separate appointment for a GP Management Plan can be made.

A GP Management Plan helps you manage your chronic or ongoing health conditions as it serves as a roadmap for your care, outlining treatments, services, and goals designed to support your overall health and wellbeing.

The comprehensive support provided by your GP and our practice nurses ensure appropriate treatment and management through patient centred discussions, active listening and shared decision making.

 

How It Works

During your appointment, we work closely with you to:

Your GP Management Plan will include recommendations for treatments, medications, lifestyle changes, and ongoing monitoring to help you manage your condition more effectively.

 

Working With a Team of Health Professionals

A key benefit of having a GP Management Plan is coordinated care.  Our nurse(s) may assist in the management of your chronic condition(s) on behalf of your GP, in line with your GP Management Plan.  Our nurse(s) can help connect you with other healthcare providers — such as physiotherapists, dietitians, podiatrists, or psychologists — to ensure all aspects of your health are supported.  This team-based approach helps everyone involved in your care stay informed and work together toward your health goals.

 

Medicare Rebates

If you have an eligible GP Management Plan, you can access up to five Medicare-rebated sessions with allied health professionals.
These sessions can help you receive ongoing support, improve your condition management, and stay on track with your treatment plan.
