What Is A Health Assessment?

A Health Assessment is a comprehensive check-up that involves an initial consultation with one of our skilled nurses who will evaluate your overall well-being, ask you questions about your health, perform a series of simple tests and screenings such as height, weight, blood pressure, waist circumference, check if you are due for any immunisations or other tests and may provide you with education on various health topics.

The nurse will then share these findings with the doctor who will then discuss these further with you and may make clinical recommendations.

As a Health Assessment is tailored to your age, lifestyle, and medical history, it provides an opportunity for personalized health advice, early intervention to improve long-term health outcomes and detect early signs of chronic conditions.

 

Types of Health Assessments

  • Chronic Disease Risk Assessment: For individuals aged 45-49 years who are at risk of developing a chronic disease, focusing on early detection and prevention
  • Diabetes Risk Assessment: For those aged 40-49 years at high risk of developing type 2 diabetes, with screenings for blood sugar levels and lifestyle factors
  • Over 75 Assessment: For individuals aged 75 years or older, focusing on overall health, mobility, and the prevention of age-related conditions
