A Mental Health Care Plan (MHCP) is a structured plan developed by your doctor to help you manage and improve your mental health. It provides a clear pathway to access the right support, treatment, and resources tailored to your individual needs.
We offer personalised Mental Health Care Plans for individuals experiencing challenges such as depression, anxiety, stress, or other mental health concerns.
During your appointment, your doctor will:
- Discuss your current symptoms and experiences
- Assess your overall mental health needs
- Work with you to set achievable goals for your wellbeing
- Develop a plan outlining the most suitable treatment options and supports
With a Mental Health Care Plan, you may be eligible for up to 10 Medicare-rebated sessions per year with a qualified mental health professional such as a psychologist, social worker, or counsellor.
Ongoing Review and Support
